Certain indicators of vitamin B6 metabolism were investigated in six patients with acquired refractory sideroblastic anemia who were only partially responsive to pyridoxine therapy. Two indexes of vitamin B6 metabolism were assessed: [1] the in vivo conversion of pyridoxine (20 mg, intravenously) to pyridoxal phosphate was estimated by determination of serum pyridoxal phosphate levels from 0 to 300 min. [2] Erythrocyte and hepatic pyridoxal-phosphokinase activity were estimated from pyridoxal phosphate concentrations generated by incubation of erythrocyte hemolysates and liver tissue extracts incubated with equimolar adenosine triphosphate, pyridoxal and pyridoxine and Mg++ at 37 C for 0, 30, and 60 min.
