The past two decades have been a time of change in medicine. There has been a great increase in research, with a consequent explosive growth in medical knowledge. This, in turn, has led to a need for increased specialization. At the same time there has been an insistent demand on the part of the people of the United States for better medical care and sharp criticism of the failure to train physicians with an interest in the provision of primary medical care.
