To the editor: In his criticism (1) of the problem-oriented medical record (2, 3), Dr. Alvan Feinstein states that better use of the standard source-oriented medical record would suffice, yet he carefully limits his remarks to in-patients at medical centers and, to the best of our knowledge, does so without having used this record in an on-going study to test his hypotheses.
Most people receiving medical care do not fall into that group. Many are cared for by several physicians—who may not have detailed chart rounds, may see
