To the editor: I read with interest the article, "Islet Cell Tumors: Current Concepts and Management," by Schein and associates (Ann Intern Med 79:239-257, 1973). Their scholarly discussion and presentation of various side effects of streptozotocin were impressive. We have used streptozotocin in a case of malignant insulinoma with multiple liver metastasis. In our case, there was severe hypoglycemia, successfully treated with streptozotocin administered on the same drug schedule as that described by Schein. High levels of calcium present before starting streptozotocin therapy reverted to normal levels after therapy. A review of the literature showed three other cases in which
