Recent interest in the functions and structure of the health care system in the United States has focused chiefly on hospital facilities for acute care, on arrangements for care of the ambulatory patient, and on the desire for more "preventive medicine" by clinical practitioners. Chronic care has received some attention from Federal funding agencies that administer Medicare and Medicaid legislation but has not been a nidus of innovative planning by the clinical community or its professional organizations.
