To the editor: The paper by P. B. Lesser and A. L. Warshaw, in the June issue (Ann Intern Med 82:795-798) is titled "Diagnosis of Pancreatitis Masked by Hyperlipidemia." This title, the abstract, and the discussion imply that by the use of "the simple amylase/creatinine clearance ratio, and the serial dilution of hyperlipidemic serum" seven cases of acute pancreatitis were "unmasked." Review of the clinical data shows that in five of the seven patients urinary amylase measurements on at least one occasion early in the clinical course were greater than six times normal.
