To the editor: In the "letter" by Matas and colleagues (1), the investigators did not mention a third possible, and perhaps more important, reason for the elevation in serum creatinine observed as blood glucose increased in diabetic posttransplant patients: relative volume depletion secondary to osmotic diuresis.
