To the editor: We were interested to read the paper, "Radiation Therapy of Midline Granuloma" by Fauci, Johnson, and Wolff (Ann Intern Med 84:140-147, 1976). They report, however, a concept and experience at variance with ours. Granulomatous lesions of the nose, as we have seen them, have usually been conveniently classifiable as either necrotizing granuloma with vasculitis consistent with Wegener's granulomatosis, polymorphic reticulosis ( 1 ), specific infectious granuloma, or lymphoma. The non-specific pathologic features referred to in the paper have suggested to us either that the biopsy was inadequate or that the disease process has not yet evolved into
