To the editor: I have just read the paper by Lyle and associates (Ann Intern Med 84:594-601, 1976). I must take issue with the statement on page 598, "Financial incentives to the physician himself may have little bearing on this behavior." The authors rightly comment that it is theoretically possible to generate as much income by having a high number of encounters with individual members of a small patient pool as it is by having a lower number of encounters with a larger pool of patients. There is an implication that the latter can be accomplished if physicians use telephone,
