To the editor: Hull and Sarubbi recently reported a practical strategy for adjusting both the dose and interval in giving gentamicin and described their success in using this strategy clinically (Ann Intern Med 85:183-189, 1976). However, use of the Siersbaek-Nielson nomogram (1) and the maintenance dose table in their Figure 4B as described could result in significant accumulation in large patients and underdosing in small patients. A 70-kg patient with a creatinine clearance (CCr) of 30 ml/min will be expected to have identical peak serum levels after each dose. The maintenance dose from their tables for a patient with a
