To the editor: I read with interest the recent paper by Stross and colleagues on the effectiveness of coronary care units in small hospitals (Ann Intern Med 85:709-713, 1976). As the sole internist in a small 40-bed hospital located at least an hour from the nearest urban medical center, I was particularly concerned by the finding of increased mortality in units seeing less than 60 patients per year. The authors were able to identify retrospectively some of the factors responsible for this finding and that corrective action had been taken. As we admit only three or four cases of myocardial
