To the editor: We have read "Spontaneous Retroperitoneal Bleeding in Patients on Chronic Hemodialysis" by Milutinovich, Follette, and Scribner (Ann Intern Med 86:189-192, 1977) with interest. The statement made by the authors on page 191 that "there have been no reports of spontaneous retroperitoneal bleeding in patients receiving chronic hemodialysis treatment" is incorrect. Vanichakornkul and co-workers (1) have reported this relatively rare complication associated with hemodialysis. Our similar experience with this complication of hemodialysis will be reported in the near future. In the differential diagnosis of spontaneous retroperitoneal bleeding, one should also consider spontaneous perirenal hemorrhage as reported by Tsai
