To the editor: The paper about the cost-effectiveness of therapy for pharyngitis by Tompkins, Burnes, and Cable (Ann Intern Med 86:481-492, 1977) is certain to arouse controversy both about how to treat sore throats and about whether to use the decision analysis strategy in clinical medicine. The accompanying editorials have paid great attention to the problem of choosing the numerical quantities for the probability values at the branch points in the decision tree; however, neither the authors nor the editorialists sufficiently stress an even greater problem that is inherent in the strategy of decision analysis and that cannot be resolved
