To the editor: We have read "Pleural Mesothelioma: Clinical Features and Therapeutic Implications" by Legha and Muggia (1) with interest. Recently, successful pleural injection of BCG or BCG cell-wall skeleton (BCG-CWS) was reported in the treatment of minimal residual tumor after pulmonary resection for lung cancer (2) and metastatic cancer to pleural cavity (3). The treatment of diffuse pleural mesothelioma presents problems, but local immunotherapy with intrapleural injection of bacterial preparations seems to be a hopeful therapeutic procedure. Direct contact of bacterial antigens and mesothelioma cells can be expected to have an antitumor effect because the mesothelioma cells tend to
