To the editor: Dr. Arieffs comments on my letter (Annals of Internal Medicine, September 1979, p. 496) repeat his misleading misinterpretation of two lines in my paper (1), which, he declares, describe the picture of aspiration pneumonia. They read: "The lungs showed a histological appearance of patchy pneumonia with desquamation of bronchial epithelium" (1).
This is not the picture of aspiration pneumonia, which has the following characteristics (2): the presence of foreign bodies; necrosis and putrefaction of lung; infiltration of polymorphs, lymphocytes, macrophages, giant cells. This histologic picture was not present, and Arieff and Schmidt's statement (3) that these rats
