To the editor: The Reilly and Reilly (1) calculation of Veterans Administration (VA) outpatient care cost was an important contribution to the analysis of public care, but their work requires debate as well as replication. Two sources of bias could be operating to increase per-visit salary costs. First, physician salaries are allocated to outpatient costs based on the 4-hour clinic sessions they are to attend. In the various institutions with which I have been familiar, physicians are rarely present in clinics over 3 hours.
