Diabetic patients with proliferative retinopathy have been shown to develop fewer episodes of transient visual loss from vitreous hemorrhage if they have received panretinal laser or xenon photocoagulation in the manner described by the Diabetic Retinopathy Study Protocol. Unfortunately, some patients never receive laser treatment before developing severe vitreous hemorrhage or tractional retinal detachment, or both complications. Of the patients who receive photocoagulation, 15% to 20% nevertheless deteriorate further. If, consequently, nonclearing vitreous hemorrhage or tractional retinal detachment involving the macula occurs, then vitrectomy surgery is the only course of treatment that will be beneficial.
