To the editor: In his recent article Dr. Eisenberg (1) broaches an important subject that faces our current health care delivery system: How to adapt for further medical needs and cost restrictions. The gatekeeper concept, using a "primary care physician" as the initial medical contact with subsequent specialty referral, has the potential to improve physician-patient relations, reduce costs, and better utilize our health care resources. The points we wish to raise with Dr. Eisenberg concern who should fill the role of the gatekeeper and what should be the role of the internist.
