To the editor: Thrombocytopenia is a known complication of infection with the human immunodeficiency virus (HIV). The mechanism of the thrombocytopenia is uncertain, although Walsh and colleagues (1) found circulating immune complexes bound to the surface of platelets of HIV-positive patients, and Stricker and colleagues (2) found specific autoantibodies bound to a platelet membrane antigen. Thrombocytopenia has recently been reported after heterosexual transmission of HIV (3). We report a case of thrombocytopenia in a heterosexual married couple who were HIV-positive. The husband probably contracted the HIV infection from contact with prostitutes, although his wife developed thrombocytopenia first.
