To the Editor: In a recent article(1) on the integrated inpatient management model, Dr. McMahon and colleagues clearly outlined some of the problems facing hospitals in the United States. In addition, they suggested a management and physician participation model to address the problem of cost containment. Although this model shows promise, I believe its success may be limited. On the management side it will be limited to academic institutions that have a "captured" or financially bound medical staff. Most community hospitals have medical staffs comprising privately practicing physicians who represent their own economic entity. I believe it would be difficult
