Purpose: To identify the sources of dysfunction of the U.S. health care system and to propose a Guaranteed HealthCare Access Plan for all U.S. citizens.
Content: This book offers a highly readable discourse on the failings of the U.S. health care system, such as relatively poor health outcomes for the level of investment, high administrative costs, and fragmentation of care. The author calls for a voucher, or health care certificate, which would guarantee every U.S. citizen benefits equal to those that members of Congress receive now. Replacing the employment-based insurance system, the plan maintains choice of private health plans and providers, eliminates means testing for eligibility, and stops the constant churning of enrollees in Medicaid. The plan would create a National Health Board with 12 regional boards, an Institute for Technology Outcomes and Assessment, and Centers for Patient Safety and Dispute Resolution—key elements for ensuring efficient, high-quality health care. The author compares his proposal with others according to 7 health care reform goals. This type of reform comes at a price: The plan proposes a 10% dedicated value-added tax (VAT) on goods and services.
