Intensification of treatment for CAD
You didn't comment on what you would do--perhaps a statin and aspirin? Also I wonder if the hemoglobin A1C of 5.6 is now too low and if you would back off on the dose of metformin?

Donepezil
Donepezil's GI side effects often outweigh the benefits, would suggest a trial and consider discontinuation if it is not helping cognition and causing GI or cardiac side effects. As a geriatrician, I have not seen much benefit
